The Supporting Independence Programme is tailored to prevent, support and manage the health and wellbeing needs of anyone eligible over the age of 18, who may be showing signs of reduced independence.

With up to 12 weeks of one-to-one support, our team of dedicated Wellbeing Advisors are committed to providing support, knowledge and resources needed to make wellbeing a priority. Initial appointments must be carried out face to face in a SIP clinic (see clinic map below), with follow-up appointments either by phone or face to face in a clinic or community setting.

Examples of support include:

• Opportunities to meet with an Advisor to 'Walk and Talk' with the aim of becoming more active and increasing socialisation
• Support to access social opportunities within the local community to reduce isolation, including volunteering and clubs, groups and events to suit hobbies and interests
• Identification, support and referrals as appropriate to support daily life including housing support and money advice
• Identification, intervention and support for low-level mental health and emotional difficulties
• Advice, information and support on lifestyle issues such as nutrition, smoking and alcohol use
• Identification of other issues such as blood pressure or use of multiple medicines and referral where appropriate including to memory clinics, continence services or community pharmacies

Each Wellbeing Advisor has an allocated area across the county where they hold 1:1 clinics for their customers. Most clinics are run either weekly or fortnightly, with up to an hour for each appointment. These could take place in various settings, from structured clinics to ad hoc appointments. These could include our Walk and Talks, support with attending appointments and local groups as part of a warm handover.

Our aim is to encourage our customers to access all areas of their local community as independently as possible.
